REALife student ministries exists to supplement the spiritual training that students receive at home.  While we realize that some students come from homes where this training does not happen, or where students are trained differently, we hope to encourage you to take an active role in the spiritual development of your child.

Medical Release 2007 - 2008 - This is the medical release form that we will need on file for each student who goes on an overnight trip with us.  Please print it out front and back on one sheet of paper.  We will be happy to notarize it for you in our office.
